Two Dogs Petsitters will not walk your dog(s) off leash for any reason. We highly recommend keeping a Keep Safe Collar and current I.D.
tags on your pet(s) at all times. We highly recommend having your pet(s) implanted with the Home Again microchip. Ask your sitter for more
information on the above listed products.
I agree that I have requested that Two Dogs Petsitting walk my dog(s). I request Two Dogs Petsitting take my dog(s)
to the dog park listed here ______________________ for _______ minutes/visit. My dog(s) is/are current on the
following vaccines: DHPP, Bordatella, and Rabies. I will provide proof of current vaccines. I am aware that there are
certain risks of injury to my dog(s) when around other dogs and have provided Two Dogs Petsitting with all veterinary
and emergency information should such an injury occur.
I agree to pay the charges accrued for the services provided as outlined in this agreement.

I understand that payment is due on or prior to the first visit.
Accepted forms of payment: cash or check made out to Two Dogs Petsitting
*We are not responsible for any injuries or illnesses that occur while your dog is in our care, but will provide the best care possible at all
times. If your dog lives or has unsupervised access to the outdoors, we are not responsible for the safety of your dog while outdoors including
but not limited to ingesting harmful materials, injury caused by other animals, injury/wandering due to unsecured fencing/gates, etc. We
strongly recommend inspecting all areas your dog has access to before you leave. Two Dogs Petsitting reserves the right to refuse service to
any dog(s) showing any signs of aggression. Two Dogs Petsitting reserves the right to refuse to walk your dog(s) if difficult, uncontrollable or
aggressive. We will use our discretion based on our experience.
